Human Trafficking Information Block Request

If you are a victim of human trafficking, you have the right to request that OneVeris block adverse information from your consumer report that resulted from trafficking.

To submit a request, please provide:

1. Proof of Identity

Information sufficient for OneVeris to verify your identity and locate your consumer file, such as:

  • Full legal name and any previously used names
  • Current and/or recent address
  • Date of birth
  • Social Security number
  • Additional identification, if needed

2. Trafficking Documentation

Documentation showing that you are a victim of trafficking. This may include a determination made by an authorized Federal, State, or Tribal governmental entity; a court of competent jurisdiction; an authorized non-governmental organization or human trafficking task force; or a qualifying signed or certified self-attestation.

3. Information You Want Blocked

Identify the specific adverse information on your consumer report that resulted from trafficking and that you are requesting OneVeris to block. You may provide this information in a written statement.

4. Preferred Contact Method

Provide either an email address or mailing address where you can safely receive communications regarding your request.
